1. Demand Trend
□ Medical Device Industry
○ The Egyptian medical device market is fairly small compared to other countries in the Middle East and Africa (MEA) region and is projected to register one of the fastest 2013-2018 compound annual growth rates (CAGRs) in the region. Per capita medical device expenditure is low, which indicates the enormous potential for growth as the country has the second largest population in the region. Low per capita medical device expenditure indicates the enormous market potential in the long term. Opportunities exist for medical companies to increase sales and market share. The market is expected to record a 2013-2018 CAGR of 11.8%, being one of the highest CAGRs in the region, reaching $845 million USD.
○ As production is limited, the market is reliant on imports and the public sector needs modernization; therefore opportunities exist for medical device companies which are prepared to operate in a climate of political uncertainty. The vast majority of the market is supplied by medical device imports, particularly from Germany, the Netherlands, the USA, China and Japan. In US dollar terms, imports increased by 23.0% in the 12 months up to September 2013, reaching US$ 561.4 mn. Diagnostic imaging and other medical devices remained the largest import product areas.
○ In US dollar terms, the medical device imports increased by 8.9% in the three months up to September 2013, reaching $126.3 million USD, with growth led by diagnostic imaging and other medical devices. In January 2013, the Health Minister, Mohamed Mostafa, announced plans to build or refurbish around 70 hospitals between 2013 and 2014. He also acknowledged that 200 hospitals and 3,800 clinics needed renovation. The new government has allocated 27.4 billion EGP to the healthcare sector in the 2012-2013 budget, which represents a rise of 18% in local terms over the previous fiscal year.
□ Blood Filter Device
○ Leucoreduction of blood components which is a process by which leucocytes are removed from blood by specific white blood cells (WBC) reduction filters. There are two types:
- Pre-storage filtration: each unit of whole blood is filtered before storage.
- Bedside filtration: it is carried at the beside of the patient with a different type of filter.
○ Since 1999, medical device companies started introducing leukocyte filters and shifting the market from bedside filters to laboratory leukocyte filters. In Egypt, leukocyte reduction can be achieved by various techniques including:
- Centrifugation
- Leukocyte filtration
- Sedimentation
- Washing
- Freeze-thawing
- Apheresis

2. Competitive Trend
○ Egyptian companies usually tend to import blood filter equipment from the European countries and the United State of America. Some of these European countries include Germany, France, Italy, Switzerland, and Spain. Many Egyptian companies prefer to import from the European countries for their high quality and relatively compatible prices. On the other hand, Egyptian companies also imports blood filter equipment from China, Japan, and Taiwan as well, for their good quality and reasonable prices.
○ The competition in the Egyptian market is not by brands mainly, because there is no brand recognition or preference to any of the brands in the field of blood filter equipment. However, the competition is according countries of origin.
○ Concerning the blood filter equipment, the following table illustrates two of the major blood filter equipment, available in the Egyptian market, and has local market presence:

○ Egyptian buyers shared some of the competing brands in Egypt, however, they added that the remaining 70% market share belongs to other brands from different countries. The world leading brands, including Pall Corporation, Macopharma, Fresenius Kabi, Hemonetics, Terumo, and Asahi Kasei, have local presence in other Middle East and North African countries such as the United Arab Emirates, Lebanon, and Tunisia.
3. Pricing Trend
○ The medical equipment companies said that the prices of blood purification filters differ according to many variables such as:
- White blood cell reduction efficiency
- Different leukocyte reduction technologies
- Performance (residual leukocytes/unit)
- Quality and consistency
- Ease of use
- Timing (how long will it take to finish the filtering process)
○ Reference to sales department of Fresenius Kabi Egypt which is a leading provider of a variety of transfusion products and devices and manufactures equipment and devices used by blood banks and hospitals for blood transfusions and donations, the price of blood purification and dialysis treatment device is 85,000 EGP.
4. Distribution System
○ Most foreign firms operate through local agents. Appointing a local agent is highly recommended and is a pre-requisite for participating in public tenders. The majority of agents are based in Cairo. However, the distribution sector is highly fragmented and many firms choose to appoint different local distribution agents to cover different regions or product ranges. For example, Johnson & Johnson (J&J) has operated its own Cairo office since 1998, but uses three local agents to distribute its products. Generally, the distribution or supplying network of the medical devices and equipment can be presented through the following diagram:
○ Tenders are widely used in both the public and private sectors for the procurement of high-value device orders and are generally considered to be fair and transparent. All of the Egyptian Ministry of Health and Population (MOHP) purchases are made through international tenders, with the most important factor taken into account being cost. Having said this, US government agency USAID is a significant investor in Egypt, having provided US$14.83 billion in assistance between 1975 and 2006. Medical device contracts backed by USAID funding must guarantee preference for goods manufactured in the USA.
